  • Medicaid - Maryland
    HealthChoice is the name of the Maryland’s statewide mandatory managed care program. The HealthChoice Program provides health care to most Medicaid participants. Eligible Medicaid participants enroll in a Managed Care Organization (MCO) of their choice and select a primary care provider (PCP) to oversee their medical care.

    2-1-1 provides free and confidential information and referral. Call 2-1-1 for help with food, housing, employment, health care, counseling and more. Learn more about your local 2-1-1 by looking it up here.

  • Clinica Proyecto Salud

  • Priority Partners
    Low and no-cost health care for Maryland residents.

  • Mid-county United Ministries
    MUM assists clients to obtain needed prescriptions for a short-term period. They are then referred to community agencies for long-term prescription needs.

  • Montgomery Cares
    Montgomery Cares provides basic medical services for people who do not have, and cannot get, insurance. The program pays part of the cost of health services, but a person may also have to pay something. Contact the clinic to find out what payments may be required.

  • Mission of Mercy
    Mission of Mercy provides free healthcare, free dental care, and free prescription medications to the uninsured, underinsured, and those who “fall through the cracks” of the healthcare system. Mission of Mercy is an independent nonprofit 501 (c)(3) that can provide healthcare without any pre-qualifications (e.g. patients do not need to prove their poverty or residency.

  • Kids' Health
    Includes resources for parents, children, and teens who want help and ideas for practical parenting, wellness, video games, emotions and behavior, growth and development, and more.

  • HealthChoice and State Programs Contact Information

    Located in Baltimore and Greenbelt, this facility offers coordinated care, including primary care, laboratory, group programs, radiology services, infusion services, nutrition counseling, prescription delivery, mental health services, and social services/outreach.   • Greater Baden Medical Care
    Greater Baden Medical Services, Inc. (GBMS) is a non-profit 501(c) 3 organization that provides primary and preventive care services for both insured and uninsured patients. Complimentary transportation is provided to and from Greater Baden Capitol Heights and Brandywine locations for qualified candidates.   • Potomac Community Resources
    PCR provides therapeutic, recreational, social, and respite care programs for teens and adults with developmental differences. We offer a wide variety of options to appeal to and be appropriate for as many people as possible.

  • Disability Rights Maryland
    Disability Rights Maryland (DRM) is federally mandated to advance the civil rights of people with disabilities. They provide free legal services to Marylanders of any age with all types of disabilities (developmental, intellectual, psychiatric, physical, sensory, learning, traumatic brain injury), who live in facilities, in the community or who are homeless.

  • Children and Youth With Special Health Care Needs Resource Locator
    Comprehensive database of resources including child care and respite care, community support services, financial assistance, home care services, therapeutic services, transportation, and information on youth transition to adulthood.

  • Montgomery County Aging and Disability Resource Unit
    To get information and or assistance for seniors, people with disabilities and or caregivers, call 240-777-3000. Information on hours, documentation, and FAQs can be found on the website.

  • Eldercare Locator
    The Eldercare Locator is a public service of the U.S. Administration on Aging connecting persons to services for older adults and their families. They can also be reached at 1-800-677-1116.

  • Long Term Care
    The National Clearinghouse for Long-Term Care Information Website provides information and resources to help individuals plan for future long-term care.

  • Centers for Independent Living
    The website provides information and links to the Centers for Independent Living (CILS), nonresidential private nonprofit agencies that are designed and operated within a local community by individuals with disabilities and provide an array of independent living services.

  • Protection and Advocacy Agencies
    This page provides information and links to the 57 P&As across the United States that are dedicated to the ongoing fight for the personal and civil rights of individuals with developmental disabilities. P&As provide legal support to underserved populations to help them navigate the legal system to achieve resolution and encourage systems change.

  • ARCH National Respite Network and Resource Center
    Respite is planned or emergency care provided to a child or adult with special needs in order to provide temporary relief to family caregivers who are caring for that child or adult.

  • Family Caregiver Alliance
    Family Caregiver Alliance is a public voice for caregivers. FCA supports and sustains caregivers with national, state, and local programs and resources such as National Center on Caregiving and Family Care Navigator. Services, education programs, and publications are developed with caregivers’ expressed needs in mind, to offer real support, essential information, and tools to manage the complex and demanding tasks of caregiving.

  • Shady Grove Pregnancy Center
    The Shady Grove Pregnancy Center offers a variety of services to anyone facing an unexpected, unplanned or unwanted pregnancy. This center offers pregnancy testing, pregnancy counseling, pregnancy classes, maternity and baby items, and counseling by volunteers trained in pregnancy counseling.
